An ADA accessibility evidence report should show the scope and date of testing, pages and templates covered, each confirmed issue with its WCAG criterion and severity, the responsible component, repair date, and passing verification. It should also disclose which checks were automated and which were manual. That lets counsel understand the program without mistaking a scan for full conformance.

Make the timeline readable

List findings, assignments, fixes, and retests in chronological order. A reviewer should be able to see that monitoring was continuous and that serious issues were handled promptly, rather than reconstructed after a claim.

Preserve the underlying proof

Link each summary line to the URL, selector, test result, screenshot or recording, and code or vendor ticket where available. The report is useful because the evidence underneath it can be inspected.
